FURLOUGH ALLOWANCE

 

¯  Full-time employees who have completed 2 or more years of service from Hire Date, will be paid 1 week of pay for each completed year of service based on a straight time hourly rate multiplied by 40 hours up to a maximum of 15 weeks at regular pay periods until all such pay credit is used.

INSURANCE COVERAGE

 

            LIFE INSURANCE

 

¯  Company will pay company portion for a period of time equal to the sum of the duration of the furlough allowance and 90 days for full-time employees.

 

¯  Company will pay company portion for 90 days from their date of furlough in October 2001 for part-time employees.

 

¯  Must pay employeeÕs portion of the cost of insurance during the extension period.

 

¯  May convert to an individual policy equal to the amount of current coverage.

 

¯  Must convert within 31 days from layoff date.

VOLUNTARY GROUP ACCIDENT and LONG TERM DISABILITY

 

¯  Coverage ends on date of furlough

 

¯  Not convertible to an individual policy.

 

MEDICAL AND DENTAL INSURANCE

 

For Full-Time employees:

 

The company will continue to pay the CompanyÕs portion of the applicable medical and dental insurance for a period of time equal to the sum of the duration of the furlough allowance and 90 days.

 

For Part-Time employees:

 

The company will continue to pay the CompanyÕs portion of the applicable medical and dental insurance for 90 days from their date of furlough in October 2001.

 

¯  You are responsible to continue payment of the employeeÕs portion of the cost of the applicable medical and dental insurance during this period.

 

¯  After the 90-day period, you will be given the chance to continue your medical and/or dental coverage (known as ÒCOBRAÓ) for up to an additional eighteen (18) months, by paying the full cost of the coverage plus a 2% administrative fee. You will be contacted within 45 days after the date of your layoff regarding the cost of continuing your coverage and how to enroll for it.

VACATION PAY

 

¯  Lump Sum Payment for all accrued, unused vacation.

 

¯  Deductions will be taken out for Federal, State and Social Security taxes only.

 

¯  No Credit Union loan and/or savings deduction, no 401(k), Flexible Spending Accounts, life insurance, or long-term disability deduction.

 

FLEXIBLE SPENDING ACCOUNTS

 

            HEALTH CARE FLEXIBLE SPENDING

           

¯  Payroll deductions cease on date of layoff.

 

¯  Not eligible to contribute to account.

 

¯  Eligible for continuation-of-coverage through COBRA. 

 

¯  May submit claims for reimbursement, the expense must be incurred for services performed in the year you deposited money into the account.

 

DEPENDANT DAY CARE FLEXIBLE SPENDING

 

¯  Payroll deductions cease on date of layoff.

 

¯  Not eligible to contribute to account.

 

¯  May submit claims for reimbursement at any time during the calendar year, even after the termination date.

 

SPENDING ACCOUNT

 

¯  Money not used for expenses incurred for services performed by the end of the calendar year will be forfeited according to IRS rules.

 

¯  Must submit expenses for services in the previous year by March 31.

 

401(k) PLAN

 

¯  May leave contributions and earnings in Plan up to age 70 ½, or may withdraw entire account balance after layoff.

 

¯  Must pay off existing loans in full within 60 days of layoff.

 

¯  If loan not paid off, it will be considered a taxable distribution and subject to an IRS tax penalty.

 

¯  Contact Fidelity Service Center for US Airways for loan withdrawal info or request a payoff balance through the automated phone system at 1-800-354-3412.

RETIREMENT

 

¯  Shortly after the furlough date, the Fidelity Service Center for US Airways will send you distribution info for the US Airways, Inc. Employee Pension Plan and the US Airways, Inc. Employees Savings Plan.

 

¯  May initiate a distribution from the program no less than 30 days following your furlough date.

 

¯  If balance is more than $5,000, you may defer distribution until a later date, but not beyond age 70 ½.

 

¯  Distribution subject to Federal and State taxes.

PASS-TRAVEL

 

¯  You and your eligible family Members currently on your travel privileges are eligible to use unlimited passes on US Airways for 90 days following layoff.

 

¯  Not eligible for interline pass travel.

 

¯  Must turn in travel card on last day at work.

 

¯  Travel for you and your eligible family Members must be completed within the

90-day period.

 

¯  Call the Employee Travel Office at 1-800-872-4780; prompt 4, for travel procedure questions.

 

 

SENIORITY/RECALL RIGHTS

 

¯  Seniority and recall rights will be done in strict adherence to the Collective Bargaining Agreement.

 

CREDIT UNION

 

¯  May leave savings & IRA accounts on deposit.

 

¯  Contact the Credit Union directly at 1-800-926-0003 to make arrangements for loan payments.
